What is Neuralign S™?

Beyond the "Zap and Hope" Approach: What Makes Neuralign S® Different from Traditional E-Stim

If you've been through physical therapy before, you're probably familiar with traditional electrical stimulation. It uses pads that deliver a preset pulse at a preset intensity regardless of what you're actually doing in the moment. It has its place, but it's fundamentally passive: the machine does the same thing whether you're moving perfectly or not moving at all.

Neuralign S®, developed by Alyve Medical, takes a different approach. Instead of stimulation happening to you on a fixed schedule, it happens in response to how you move. This is motion activated stimulation, from an FDA cleared device that can be used in the office or rented at home via prescription from your physician. Alyve Medical is a company that I consult with to help develop and improve their devices, and to help get technologies that truly help patients in to the hands of the proper people. Here is my disclosure- this company pays me to help improve their devices. I will only ever use them with patients that, in my experience, will actually benefit. And I will only ever do so with a business model that is ethical and affordable for the value it provides.

What Neuralign S® (The “Shoulder Pacemaker”)Actually Is

Neuralign S® is a wearable neuromuscular electrical stimulation (NMES) device designed for the treatment of shoulder pathologies. It is often paired with Showmotion®, a specialized software that helps shape the treatment process. Built into the device is a kinematic sensor (i.e. accelerometer), so the patient's own movement — rather than a preset timer — is what activates the stimulation, using the device's patented algorithm.

That's the core distinction from traditional stimulation devices: the shoulder pacemaker dynamically interacts with the patient during active rehabilitation. Using its sensor technology, it detects a patient's movement during therapeutic exercises and responds by progressively stimulating the targeted muscles in real time. This is a concept the company calls Motion Activated Stimulation, or MAS.

How It Works in a Session

This device is controlled through a tablet app via wireless connection. The app walks the you through each exercise with instructions, required repetitions, and proper electrode positioning. There are multiple therapeutic protocols built for different shoulder pathologies, which a clinician can customize or manually adjust for an individual patient.

While the device is in use, the app tracks repetitions, monitors the patient's progress, and provides real-time visualization of the exercise for both the patient and the therapist to follow. That combination of feeling the stimulation, seeing it on screen, and hearing feedback cues gives the sensory, auditory, and visual biofeedback needed to reestablish proper physiological muscle activation through what's described as a "feed-forward" mechanism. This is how the nervous system relearns the correct movement pattern. This is fundamentally different than just the muscle getting a jolt, and it is why this device has its own patent.

Who It's Built For

The device is used for shoulder motion deficits and muscle activation deficits. This can be used for pre-habilitation ahead of surgery, rehab following a surgical procedure, a non-surgical intervention for a long-standing shoulder issue, or conditioning for athletes trying to restore optimal joint mechanics. It’s very effective for joint instability in many cases- chronic dislocations, feeling like you have to move your shoulder in a specific way or it “clunks”. It's not limited to a single diagnosis; it applies broadly to conditions involving diminished or altered shoulder movement. A painful arc of motion due to impingement, weakness with overhead activity, a biceps tendon with subluxations… this device is worth at least a brief trial in many cases of shoulder pain. There is often a noticeable improvement in symptoms, or an objective improvement in movement quality as seen via the Showmotion® movement analysis system.

It's Backed by More Than Marketing Language

This device is more than just a clever engineering idea, it's been through actual clinical scrutiny and is constantly undergoing refinement. An independent multi-center randomized controlled trial evaluated the safety and efficacy of the Shoulder Pacemaker in treating functional posterior shoulder instability, and the resulting study received a Best Paper award at the SECEC-ESSSE Annual Congress a major European shoulder and elbow surgery society. That's a meaningfully higher bar than most consumer stimulation devices ever clear. Here is a link to the study: https://pmc.ncbi.nlm.nih.gov/articles/PMC7364790/

Why This Matters for Recovery

A lot of shoulder rehab hinges on one thing: getting the right muscles to fire in the right sequence again, especially after an injury or surgery has disrupted normal movement patterns. Passive stimulation and strengthening can help with pain and general muscle activation, but it can't teach timing. Because Neuralign S® ties the stimulation directly to the patient's actual motion, it's addressing that sequencing problem directly. It is reinforcing the correct pattern at the exact moment the body is attempting it, rather than on a schedule that has nothing to do with what the shoulder is actually doing. This creates long term changes.

For patients who've plateaued with more traditional approaches, or who are working through a shoulder issue that hasn't responded the way it should, this kind of motion-driven, closed-loop feedback can be a meaningful addition to a rehab plan.
